MEDICAL SOCIAL WORKER III

Duties:
The Medical Social Worker provides comprehensive clinical social work services for children and their families to assist them in coping with and adapting to the acute and chronic health care conditions of their children.
As a member of the Hospitals interdisciplinary healthcare team, the Medical Social Worker assists the hospital in creating an environment and attitude responsive to meeting patient and family needs through understanding psychological, socioeconomic, cultural, emotional and spiritual factors influencing health and illness.

Responsibilities:
1.
Complies with established hospital customer service standards including the following:

In accordance with departmental screening criteria, provides intake assessments of families to identify and plan or treatment/intervention needs including childrens medical, developmental, and emotional status, familys functioning, and the need for community or program services.
Assesses children and families at risk for maltreatment and when indicated, reports to protective service agencies in accordance with State law and hospital policy.

Provides therapeutic interventions with patient and family including individual, parental, family and group therapies.
The interventions provided will be based on assessment of need and include crisis intervention, supportive counseling, and grief and bereavement counseling.
4.
Understands the medical, diagnostic, and treatment implications of their patient population.
Assists patients and families to understand, accept and follow medical recommendations.
Demonstrates an appreciation for the bioethical issues which may arise in a patients care.
5.
Collaborates closely with case management team to identify and resolve potential barriers to discharge; provides clinical interventions to support a safe and efficient transition to the home.
6.
Locates, coordinates and assists in determining eligibility of social, medical, financial, and other services to meet the patient and familys needs.
Secures and provides needed concrete services for families during the patients hospitalization.
7.
Documents assessment and interventions in the patients medical record in accordance with hospital and departmental standards.
8.
Conducts informal and formal family health care team conferences to ensure most efficient coordination of health care and related services.
9.
Collaborates closely with other hospital programs and community agencies with which patients and families are involved and advocates for patient and familys needs as indicated.

Participates in departmental and hospital performance improvement projects, including hospital committees and C.

Compiles weekly data collection forms through FMI tools and submits bills and supporting documentation for reimbursable services.
12.
Provides social work case management functions for chronically ill outpatient and those patients hospitalized three days or more.
13.
Ensures that services provided to families are culturally sensitive and in the patients primary language; ensures that religious and ethnic issues pertinent to the childs medical condition and plan of care are incorporated in the psychosocial assessment and the childs treatment plan.
14.
Implements and adheres to policies which ensure patient confidentiality.
Education:
Masters Degree In Clinical Social Work From A Program Accredited By The Council On Social Work Education.
Licensed By The Board Of Behavioral Science Examiners.
This is the pay range that RightSourcing (a part of Magnit) reasonably expects to pay someone for this position, however, as a supplier your expected pay range may vary and/or include certain benefits like:

May provide clinical supervision, including weekly individual supervision and emergency case consultation to Psychology Interns, Psychology Fellows and Social Work Interns as needed.
May provide didactic teaching, group supervision and seminar leadership for department trainees as assigned.
May take necessary Continuing Education courses in clinical supervision, as required by the licensing Board, in order to provide supervised hours for Psychology Interns, Fellows and/or Social Work Interns.
